So, just exactly what is urinary incontinence? Urinary incontinence (UI) is defined as the loss of urine control, or the inability to hold your urine until you reach a restroom. UI is very common, with more than 12 million people in the United States suffering with the condition. It affects both males and females, however postmenopausal women are the most likely to develop the problem. UI can range from the aggravation of slight losses of urine, to severe, frequent wetting.

Obstetrics
Gulf Coast Obstetrics and Gynecology is one of the only Sarasota offices that can offer patients in-office procedures.
Endometriosis
Dr. Garner takes a special interest in minimally invasive surgical procedures and is certified in laparoscopic and DaVinci Robotic Assisted Minimally Invasive Surgery including myomectomy, hysterectomy and complex laparoscopy for endometriosis and adhesion resection. From 2012-2016 Dr. Garner served as Chief of Obstetrics and Gynecology at Sarasota Memorial Hospital where he worked with community leaders and hospital administration to improve women’s health services in Sarasota. In 2013, he ws named Physician of the Year at Sarasota Memorial Hospital, and has now been selected to serve as Chief of Staff at Sarasota Memorial Hospital in 2018.
Pregnancy
Do not use Mirena® if you • might be pregnant • have had a serious pelvic infection called pelvic inflammatory disease (PID) • have had a serious pelvic infection in the past 3 months after a pregnancy • have more than one sexual partner or your partner has more than one partner • have an untreated pelvic infection now • can get infections easily. For example, you have problems with your immune system, leukemia, AIDS, or intravenous drug abuse • might have cancer of the uterus or cervix • have bleeding from the vagina that has not been explained • have liver disease or liver tumor • have breast cancer now or in the past • have had a tubal pregnancy or know you are at high risk for a tubal pregnancy • have an intrauterine device in your uterus already • have a condition of the uterus that distorts the uterine cavity, such as large fibroid tumors • are allergic to levonorgestrel, silicone, or polyethylene.
Labor and Delivery
Our nurse midwives are board certified to provide health care involving emotional and physical support to women before, during, and after childbirth. They are registered nurses with specialized training in assisting pregnant women and their newborn babies. They also participate in a variety of services that include providing routine gynecological services like Pap smears and breast examinations, advising women about reproductive health and personal care, and monitoring the health of the mother and fetus during pregnancy. Our CNMs also provide education for women and their families about childbirth methods, infant care, nutrition, and proper exercise. They work closely with our obstetricians when medical treatments, medications or surgical interventions are necessary.

Urology
Pelvic floor reconstruction techniques the best method to repair this myriad of support problems has been a very controversial issue in gynecology and urology for decades. Many surgical approaches have been proposed, and tried over the years, and so there is good data to help guide us in determining which method is best. Some of the procedures lead to other problems a few years later, while others have high failure rates. Some fail early while others last for a decade. Here is a list of procedures that have been used, or are still being used to correct these problems, which, if you look closely, almost all were designed primarily as a correction for urinary incontinence.
Ovarian Cancer
Please note that you should still have an annual examination for breast cancer screening and an annual pelvic examination for ovarian cancer screening.
Cervical Cancer
The Pap test, sometimes called a Pap smear or cervical cytology screening, was named for George Papanicolaou, the doctor who designed the test, and it is a simple test to look at cells taken from the cervix. Since the Pap test was developed, more than 50 years ago, it has greatly reduced the number of deaths caused by cervical cancer in the United States.
Urinary Tract Infection
What causes urinary incontinence? Although age alone is not thought to cause UI, the condition is very common in older people. This is probably because older people have had more time for disease or injury to occur, thus leading to the incontinence. UI can be caused by diseases which alter body function, the use of medications, trauma from childbirth, and/or the onset of an illness. Sometimes, incontinence is the first and only symptom of a urinary tract infection. Women experience incontinence twice as often as men. Pregnancy and childbirth, menopause, and the structure of the female urinary tract account for this difference. But both women and men can become incontinent from neurologic injury, birth defects, strokes, multiple sclerosis, and physical problems associated with aging.
Prostate Cancer
There had been concern that the risk of testicular and prostate cancer may be increased in men who have had a vasectomy, but this does not appear to be the case.

Multiple Sclerosis
Urge incontinence - Often referred to as “overactive bladder”, this type of incontinence is the inability to hold urine long enough to reach a restroom. It is often found in people who have conditions such as diabetes, stroke, dementia, Parkinson's disease, and multiple sclerosis, but may be an indication of other diseases or conditions that would also warrant medical attention.

Ultrasound
First Trimester Nuchal Translucency Testing – This test, offered between 11 and 14 weeks gestation, uses an ultrasound to measure the fluid at the back of a baby’s neck, and blood work form the mother to measure levels of certain blood proteins. The combination of these is used to predict the likelihood that the pregnancy is affected by chromosome abnormalities such as Down syndrome, Trisomy 18, and Trisomy 13, and will detect up to 85% of affected pregnancies. Remember that this blood test only tells you if you are at risk for having a baby with any of these disorders. Follow-up testing is needed to confirm the presence of these disorders.

Chemotherapy
The ability of cord blood stem cells to differentiate, or change into other types of cells in the body is a new discovery that now has been used in transplant medicine to regenerate a patient's blood and immune system after they have been treated with chemotherapy and/or radiation to destroy cancer cells. It also holds significant promise for improving the treatment of some of the most common diseases such as heart disease, stroke, and Alzheimer's. Parents now have the ability to collect these cells from the umbilical cord of the baby after birth in order to store or "bank" the cells for use at a later date.

Dr. Easterling takes a special interest in minimally invasive surgical procedures and is certified in laparoscopic and DaVinci Robotic Assisted Minimally Invasive Surgery including myomectomy, hysterectomy, pelvic suspension procedures among others. He also serves as an Assistant Professor with the Florida State College of Medicine.

Jennifer Miller was born in Sarasota, Florida. She received her Bachelor’s in Nursing from the University of South Florida and Masters in Nursing at Saint Joseph’s College of Maine. She has worked as a Registered Nurse at Sarasota Memorial Hospital and Doctors Hospital of Sarasota for 13+ years. While studying for her APRN degree she worked in Orthopedic Surgery and Cardiothoracic surgery settings.
Vasectomy
A vasectomy is a contraceptive method for men that consists of cutting or blocking the vas deferens; the tubes that carry sperm from the testes. It is similar to a tubal ligation that is performed on women, although it is an easier procedure since it can be done in the office setting and without general anesthesia. A urologist is the physician who typically performs this type of surgery.
Robotic Surgery
Hysterectomy
Menorrhagia is defined as unusually heavy or long-lasting menstrual periods, and it occurs in aproximately 1 out of every 5 women. During these heavy periods, women may experience severe pain, the passage of clots, as well as the need to change her pads or tampons very frequently. Bleeding such as this may interfere with ones ability to function at work, at play, and even at home. Causes for this heavy bleeding may include endometrial polyps, fibroids or horomonal imbalances that disrupt the normal cycle and many women are told that their only alternative to control their symptoms is to undergo a hysterectomy.
Endometrial Ablation
As a Level II Licensed Surgery Site, we can complete procedures for conditions such as abnormal uterine bleeding, abnormal pap smears, or even heavy menstrual cycles due to polyps or fibroids, can be evaluated and treated with quick procedures using hysteroscopy, the Novasure endometrial ablation or Myosure fibroid resection. These procedures offer little to no down time and often have dramatic improvements in patients’ quality of life.
Laparoscopy
Many gynecologic surgeries today are still too complex to accomplish with conventional minimally invasive technologies. Until now, these patients have required a large open incision for their surgery. Many of these surgeries are myomectomies (removal of fibroids) or hysterectomies (removal of the uterus). More than half the hysterectomies performed in the U.S. still need to be done using large incisions. The dexterity needed to remove fibroid tumors and suture the uterus properly can be challenging to impossible via traditional laparoscopy. Robotic surgical enhancements today make this possible. Robotic assisted hysterectomy now offers a progressive, dynamic alternative to conventional laparoscopic and abdominal hysterectomy.

Lesions
During pregnancy perinatal transmission to the developing fetus can occur. Primary, or first time, outbreaks of the HSV virus carry the greatest risk for transmission during pregnancy and may also have the greatest effect on the developing fetus. Later transmission can occur, however, in anyone who is actively shedding the virus. During labor, or after your water breaks, your provider will inspect the vaginal area to ensure that no lesions are present. You should report any recent symptoms of outbreaks such as blisters, tingling or burning that have occurred within the past 2 weeks. If you have had any of these symptoms, or if your provider finds any lesions consistent with herpes, they will recommend you have a cesarean section. By bypassing the vagina, this reduces the chance of the baby being exposed to the virus, and contracting the infection in the post-natal period.
