Excess loose skin and fat in the upper arms from weight loss or aging can be removed in a procedure called an arm lift or brachioplasty. Depending on the amount of skin to be removed, an incision is made in the armpit (for minimal skin laxity), or along the inner aspect of the arm (most commonly).

Description: After 2 children, this patient wore a 34AA or A bra. She had always desired larger breasts and came to her consultation wanting to do something for herself. She desired a change to a 34 small C. She looked at multiple implant sizes which she tried on with the bra she desired to fill and selected 240-260cc high profile saline implants filled to 250cc on the left and 260cc on the right which fit her narrow chest yet gave her adequate projection. She is very pleased with her selection and reports that her primary care physicians have told her she has a very natural result.

Description: This patient was diagnosed with invasive ductal (breast) cancer of her right upper outer breast. She was advised by her breast surgeon that approximately 10cm of tissue with skin would need to be removed. She also had very large 38E breasts and had considered breast reduction in the past but had never found a good time to pursue it. Dr. Lu discussed options of post mastectomy reconstruction versus a large lumpectomy with simultaneous breast reduction. (Lumpectomy without rearranging tissue would have left a significant depression and displaced her nipple areolar complex towards her upper outer breast.) The patient elected for lumpectomy and breast reduction, her tumor was completely removed and her skin was healed enough to allow her to start chemotherapy 6 weeks after surgery. 758 grams were removed from the right side and 667 grams from the left. She subsequently had radiation to the breast. She is very pleased with her result, particularly preservation of breast sensation, breast symmetry and size (38C/D), and the completion of surgery in one operating room visit.

Botox
Do you look tired, sad, or angry even when you feel well? Botox® is an injectable medication FDA approved for treatment of moderate to severe frown lines between the brows in adults younger than 65 years of age. Botox® works by blocking nerve impulses to injected muscle, temporarily stopping the ability to frown or wrinkle. Good results can also be obtained for crow’s feet and forehead wrinkles. Over time, sun damage and constant wrinkling or furrowing may cause creases in the skin itself. While these wrinkles may improve with Botox® injection, use of a skin filler may be required for additional correction. Many younger women have Botox® to prevent this condition. Over 2.4million Botox® injections were performed in 2008. Injection of Botox® is done with a tiny needle, has no downtime and onset of action begins at approximately 3-5 days after injection. Muscle activity returns at approximately 3 months. Anecdotally, some Botox® users also report improvement in their headaches after injection. This indication is currently under study.
Brow Lift
Description: This patient complained of asymmetric upper eyelids with excess skin resting on her lashes. She felt this made her look unduly tired. After discussion of both brow lift and blepharoplasty, the patient elected for blepharoplasty only. Dr. Lu performed upper eyelid blepharoplasty (upper eyelid tuck) under general anesthesia at the same time as another procedure. Results were immediate, with improved space between the lid margin and the eyelid crease for a fresh alert look and increased ease in applying eye makeup. Results could be improved with periorbital laser for fine lines on the lower eyelids.

Liposuction
Liposuction was the second most popular plastic surgical procedure performed in 2008 with over 340,000 cases in the US. Liposuction permanently removes unwanted areas of excess fat through tiny incisions placed in inconspicuous locations such as the belly button, bikini line or inferior buttock creases. Best results are obtained in patients with good skin elasticity and normal body weight with areas of disproportionate fat deposition. Dr. Lu performs tumescent liposuction by injecting the treatment areas with fluid containing small amounts of adrenaline. This fluid dramatically reduces the amount of bleeding and bruising, as does the compression foam and girdle worn after the surgery. While there has been much media attention surrounding laser assisted liposuction, to date there is no scientific evidence that laser assisted liposuction causes more skin retraction than tumescent liposuction. Laser assisted liposuction also carries additional risks of heat damage (burns) to skin and underlying tissue.
Breast Augmentation
After pregnancy, nursing, weight gain or loss, or simply with aging and gravity, breasts may sag and lose firmness and volume. The nipple and areolar complex may droop and be positioned too low on the chest, and skin may lose elasticity. Women with small breasts or loss of volume and poor breast shape may require both breast augmentation with implants and reshaping of the breast with mastopexy to achieve an optimal result. Mastopexy-augmentation can be done as two separate procedures several months apart, or at a single surgical session. The decision to perform this as a single or staged procedure is made with Dr. Lu after a physical examination and discussion of your goals.
Breast Reduction
Excessively large, drooping breasts may result in symptoms such as back, neck, and shoulder pain, skin irritation, shoulder grooving or bruising from bra straps, difficulty or pain with exercise, and arm numbness. Women report difficulty finding clothes which fit and support properly, problems with fitting into airplane or car seats, difficulty with sitting in narrow restaurant booths and feeling self conscious in a variety of situations. Breast reduction surgery removes excess skin, fat and breast tissue to produce a breast shape and size in proportion with your body and to alleviate discomfort from breast weight. The nipple and areola complex if too large may be reduced in size and is shifted into a higher, perkier, more appropriate position. In most situations, the breast tissue directly behind the nipple and areolar complex is preserved in order to keep the blood flow intact (pedicled breast reduction) with a maximal chance for nipple sensation and future breast feeding. In cases where the breast is very large, the nipple and areolar complex is very low on the chest and a small breast size is desired, it may be recommended to remove the nipple and areolar complex and replace it as a skin graft (free nipple graft). Nipple grafts do not have sensation and do not have breast feeding potential.
Breast Lift
Description: This patient complained of sagging breasts after two children. Dr. Lu performed a vertical pattern breast lift to improve the shape of her breasts and the position of her nipples. The patient continues to be happy with her results more than two years after her procedure.
Breast Reconstruction
Breast reconstruction is a physically and emotionally rewarding procedure both for the woman who has lost a breast due to cancer and for the physician as well. The creation of a new breast can dramatically improve your self-image, self-confidence and quality of life. Although surgery can give you a relatively natural-looking breast, a reconstructed breast will never look or feel exactly the same as the breast that was removed. In most cases, the reconstructed breast does not have sensation, and in the cases of unilateral (one sided) mastectomy, one or more procedures may need to be performed on the opposite breast to obtain the best symmetry. Breast reconstruction is a process which may last from months to one year, therefore, a good relationship between the plastic surgeon and patient is essential. Breast reconstruction can begin at the same time as mastectomy (immediate reconstruction) or may be delayed until you heal from mastectomy and recover from any additional cancer treatments (delayed reconstruction). Dr. Lu has a special interest in breast reconstruction and participates in the Overlake Hospital and Swedish Hospital Multidisciplinary Breast Conferences and will discuss your plan of care with you, your oncologist and breast cancer surgeon to jointly determine the optimal timing for your reconstruction. Multiple options for breast reconstruction exist, including reconstruction with your own tissue (TRAM or latissimus flap) or implant reconstruction. Many factors, such as general health, weight and body shape, prior surgeries or scars, planned radiation and personal preferences will influence your decision regarding type of reconstruction. Dr. Lu prefers to see you prior to your cancer surgery so that all options can be discussed with you and a coordinated plan made prior to any intervention.

Face Lift
Facial aging from gravity, environment, aging, weight loss and gain and genetic influences may lead to loss of facial fullness, wrinkles and skin droop. Injected skin fillers smooth and contour facial wrinkles such as the nasolabial folds (between the nose and corner of mouth), chin lines (marionette lines) or grooves between eyelids and cheeks. Fillers also plump areas which have become flattened or thinned with age such as cheek areas or lips (also known as the “liquid face lift”). Fillers were the second most common nonsurgical plastic procedure in 2008, with over 1.2 million procedures.
Blepharoplasty
Upper blepharoplasty is the most commonly requested cosmetic operation by Asian patients. Many Asians demonstrate a single eyelid, so termed because the upper lid drapes like a smooth sheet from the eyebrow to the eyelashes without interruption. Surgical creation of an eyelid crease (palpebral furrow) divides the single eyelid into two well defined segments, producing the “double eyelid". Some Asians with a natural “double eyelid” may have poor segment definition, low placement of the crease, abundant eyelid fat and excess eyelid skin which may lead to functional consequences. Dr. Lu performs her Asian upper eyelid blepharoplasty (upper eyelid tuck) under local anesthesia with or without sedation with the “full incision” approach. This involves removing excess skin and fat with precise and accurate suturing of the crease. All eyelids have some degree of natural asymmetry. Local anesthesia allows Dr. Lu to check the position of the eyelid creases during the procedure to minimize this asymmetry. This results in even, well defined creases and eyes which look more alert and open.
Abdominoplasty
A “mini-abdominoplasty” tightens the skin and muscles of the lower stomach often without relocating the belly button. This has the benefit of avoiding a scar around the belly button, but is most appropriate for women whose excess skin and muscle looseness is in the lower abdomen only, otherwise, upper abdominal bulging may result. A full abdominoplasty has two incisions, at the bikini line and around the belly button. Muscles are tightened in the upper and lower abdomen and the skin tightened throughout the entire abdomen from the bottom of the rib cage to the pubic area. The belly button is relocated to an appropriate position.
Otoplasty
Protruding or abnormally shaped ears may cause psychological trauma from the teasing of peers. Correction of protruding ears may occur at any time after age 5 as ears approach mature growth and if the child can voice their feelings. Protruding ears may result from a combination of factors, including inadequate folding of the ear back in the upper or middle 1/3 of the ears, overdevelopment of the concha or “bowl” of the ear which pushes the ear forwads, too obtuse or forward an angle at the junction of the bowl with the skull, or forward placement of the ear lobe. In most cases, an incision is hidden in the back of the ear and the ear is folded back and reshaped with precisely placed permanent sutures. If the concha or bowl is too large, an incision inside the bowl on the front of the ear may be necessary, however, it is placed as inconspicuously as possible. Wearing of a headband for several weeks afterwards is mandatory to avoid bending the ear forwards during healing.
Lip Augmentation
Description: This woman complained of thin lips with inadequate fullness, particularly in her upper lip. She desired lip augmentation. Dr Lu injected 1 cc of Restylane to her upper and lower lip.
Laser Resurfacing
Excess skin, fat and wrinkling of the upper and lower eyelids can result from a combination of factors, including aging, genetics, or environmental factors. Upper eyelid blepharoplasty removes excess skin and fat from the upper eyelids through an incision placed in the natural eyelid crease to give a more rested and youthful appearance. Over 195,000 blepharoplasty procedures were performed in 2008, making it the fourth most popular plastic surgical procedure. In the lower lids, fat bags, droopiness and skin excess can be addressed through an incision placed inside the lower lid (transconjunctival) or on the outside just under the lash line (subciliary). Additional procedures to improve the area around the eyes include laser resurfacing for fine wrinkling or injection of fillers to smooth the groove between the eyelid and cheek.
Forehead Lift
Dr. Lu will discuss these options with you in your consultation and help you select the option best for you. Many people with aging skin may have drooping skin in both the forehead and eyelid areas and may require both a blepharoplasty (eyelid tuck) and brow lift (forehead lift).

Skin Cancer
There are many types of skin cancer. Melanoma type skin cancer forms in melanocytes (pigment skin cells). Basal cell skin cancer forms in the small round cells in the outer layer of the skin. Squamous cell skin cancer forms in the flat cells in the outer layer of the skin. Rarer types of skin cancer can form in the neuroendocrine cells of the skin. Most skin cancer forms in older individuals in sun exposed areas or in patients with weakened immune systems. According to the National Cancer Institute, there were over 1 million new non melanoma skin cancers diagnosed in the US in 2009. Treatment of your skin cancer often requires surgery to remove the cancer, together with a rim of unaffected skin (the margins) to reduce the chance that the skin cancer will come back. Depending on the type, location and appearance of your cancer, Dr. Lu may recommend wide local excision (for small, well localized lesions), wide excision with frozen section controls (where a pathologist looks at the tissue removed immediately to determine whether additional tissue needs to be removed), or Moh’s excision in conjunction with a Moh’s dermatologic surgeon. If you are recommended to have Moh’s surgery, you will have your cancer removed on one day, and your wound reconstructed the following day. A small wound may be directly closed into a straight line scar. A larger wound may require repositioning of adjacent tissue to cover the wound (flap closure) or alternatively, a skin graft. Scars will be positioned to best follow natural skin lines and to minimize distortion of normal facial features and to restore a more natural appearance, however, no reconstruction is perfect and scars, tightness and distortion may persist. More than one surgery may be necessary to obtain clear margins or to perform an optimal reconstruction. The most important goal is to cure the skin cancer.

Some breast reductions may be covered under your insurance plan. Most require documentation through your primary care provider of functional problem/symptoms for greater than one year with three months of failure of conservative therapy (muscle relaxants or ibuprofen related medications, chiropractic therapy, physical therapy, yeast rash prescription creams, or custom fitted wide strapped bras). Most require a specific amount of breast tissue to be removed from each breast based upon the woman’s height and weight (see your insurance provider’s medical policy guidelines for breast reduction for specific details.) Dr. Lu can estimate the amount of breast tissue that can be removed from your breasts at your initial consultation.
