Abnormal Body Weight – Obesity immediately brings to mind associations with hypertension, diabetes and heart disease. Yet, most people are surprised to learn that there is an association between obesity and infertility. Epidemiological data confirm that obesity accounts for 6% of primary infertility, and even more surprising, that low body weight in women accounts for 6% of primary infertility. Thus, 12% of primary infertility results from deviations in body weight from established norms, and that this infertility can be corrected by restoring body weight to within normal established limits. More than 70% of women who are infertile as the result of body weight disorders will conceive spontaneously if their weight disorder is corrected through a weight-gaining or weight-reduction diet as appropriate.

Anovulation – Anovulation means lack of ovulation, or absent ovulation. Ovulation, which is the release of an egg from the ovary, must happen in order to achieve pregnancy. If ovulation is irregular, but not completely absent, this is called oligovulation. Both anovulation and oligovulation are kinds of ovulatory dysfunction. The embryo will be ready for transfer into the uterus. The embryos are placed in a tube and transferred back into the uterus. The procedure is usually painless, though some women may experience some cramping. The number of embryos transferred depends on a womanâ€™s age, cause of infertility, pregnancy history, and other factors. Post transfer requires 3 days bed rest and a two week waiting period for pregnancy testing.

Dr. Sameh Toma is one of the most experienced fertility doctors in the Raleigh area, specializing In Vitro Fertilization and in Tubal Reversal Surgery. He graduated from North Carolina State University with a Bachelor Degree in Science in Electrical Engineering. He then graduated from the University of North Carolina, Chapel Hill School of Medicine and completed his residency there as well. During his residency, he conducted significant research and published several articles regarding infertility, endometriosis and fetal wellbeing. His most significant training came under the late Dr. Luther Talbert, Fellowship Director for Reproductive Endocrinology at UNC-Chapel Hill. Dr. Talbert was well known across the state of North Carolina as he trained the majority of obstetricians and gynecologists during his years at UNC.Â In the early 1990’s Dr. Toma, Dr. Gerald Mulvaney and Dr. Talbert left UNC-Chapel Hill to open the North Carolina Center for Reproductive Medicine. Together, they created the most prolific and successful fertility clinic in the region. For twelve years, Dr. Toma worked side by side with Dr. Talbert on hundreds of patients being treated for infertility.Â That intimate training allowed Dr. Toma to take over as Medical Director of the North Carolina Center for Reproductive Medicine, per Dr. Talbert.

“Since I was 14 years old, I have had severe pain in my pelvic area. The first doctor I saw put me through test after test, finally reaching the decision that the pain was caused by an ovarian cyst and that I needed to have surgery immediately. My father called Dr. Toma that same week to ask him for his expert opinion. Dr. Toma instantly suggested I had another MRI and told us to wait for those results before doing anything. We received the results the day before I was scheduled for surgery, Dr. Toma told us that I did not need to have surgery to remove the cyst and most definitely did not need to have my ovary removed.

“I remember hearing my oldest brother tell my mother once, that he thought all people were inherently bad. This question of what people really are is a question I’d always asked myself. However, my answer came in a most unexpected way. My mother woke me up one lazy summer morning, shortly before I was due to leave home to attend college at BYU-Hawaii. I was dreaming when I heard her voice tell me to get up. However, her voice sounded strange and, as I sat up, still only half conscious, I figured it was the sleep that was making me hear things. I walked out into the hall and to the laundry room door. As I leaned against the door frame, my mother turned towards me from where she was standing, ironing clothes. I could see that her eyes were watering and as I opened my arms towards her, she came to me, accepting my hug, and started to cry. She began to tell me about my older sister, Kari. Last February, Kari was diagnosed with Hodgkin’s Lymphoma and had to undergo four months of chemotherapy before being told that she was in remission. However, in July, our family was given the unfortunate news that she had relapsed. So my mother was frantically looking for the best hospitals and the newest, cutting edge treatments for my sister. With all the doctors, the negative survival ratios, and the stress on starting treatment as soon as possible, my mother was, understandably, overwhelmed and stressed out. As my mother explained about Kari’s new treatment, she made it clear that this second round of chemotherapy would destroy my sister’s reproductive system, making it impossible to have children. However, she also told me that there was one surgical procedure that could be done to preserve one of Kari’s ovaries. With tears falling down her cheeks, my mother went on to tell me that the hospital that specialized in that particular procedure wanted twenty-five thousand dollars up front and with no guarantee that the surgery would be a success and that there was no way that our family could afford to pay that kind of money out of pocket. “We don’t know where we’re going to have to spend our money yet, because we don’t know what the insurance company will or will not pay for,” my mother said. Upon finding this out, Kari had apparently insisted that she be allowed to have the procedure done despite the costs. At this point, my mom was crying in my arms and I had no idea what to do or say that would help. I knew she was frustrated and that she felt utterly helpless to help her fatally ill child in her time of need. My mother finally regained control and managed to stop crying before going back downstairs to make some more medical calls. I went back into my room, intending to get dressed and shower before starting my day. However, upon entering my bedroom, I felt this overwhelming urge to pray. NCCRM provides the best success at one of the best prices in the industry by providing high quality service in a most efficient process. An example is how we do liposuction to decrease the abdominal thickness in patients with a higher BMI. This allows better access to the tubes. The result is a better-quality anastomosis that results in a higher chance of a successful pregnancy. NCCRM is the only fertility practice with liposuction capability and it still less expensive than most infertility clinics charge for a tubal reversal alone. This is one of the advantages that helps make our success rates the highest on the East Coast.

Laparoscopic Surgery
If left untreated, less than half of ectopic pregnancies will resolve on their own. These are the tubal abortions. The advent of methotrexate treatment for ectopic pregnancy has reduced the need for surgery. Â Laparoscopic surgery might be required should methotrexate treatment be unsuccessful or in danger of rupturing. Should your fallopian tube rupture and there is the danger of bleeding out; a more invasive procedure, known as a laparotomy, would need to be performed to stop the bleeding.
