Headaches associated with muscle spasms and those with a defined referral pattern from the neck area frequently respond to a course of Physical Therapy using therapeutic modalities such as Laser, Electrical Stimulation and Ultrasound as well as some manual release techniques along with Craniosacral treatments.
MRI
This is also known as Spasmodic Torticollis and is characterized as painful spasms and twisting of the neck with or without tremors. This is known as a movement disorder due to abnormal posturing of the neck. There are typically normal Cervical MRI findings. Patients present with painful spasms and complain on inability to move the head and neck through a normal range of motion. There is usually no weakness or numbness into the arms.
X-Rays
Computed Tomography
During this procedure, X-ray guidance is used. This is a diagnostic test to view and assess the internal structure of the disc and determine if it is a source of pain. Essentially, it is analogous to “pressing/palpating” on the disc to see if it is painful/inflamed. If so, we may inject medication (anesthetic and steroid) to ease pain and inflammation. So we can identify disc abnormalities using a CT scan, we inject contrast dye into the disc. We look for annular tears, scarring, disc bulges, and other changes to the disc nucleus. The procedure can take up to 30 to 45 minutes, plus 30 minutes if a CT scan is indicated, plus approximately 45 minutes of recovery time. A Spine Surgeon usually requests this to help determine which discs should be operated on or not.

Sciatica
Sciatica is a lumbar radiculopathy that affects the large sciatic nerve, where nerve roots come together in the buttock. Sciatica pain is a sharp, often shooting sensation that radiates down from the buttock to the back of the thigh and into the leg. It often feels like a stabbing pain deep in the buttock and grows worse with sitting.
Whiplash
A common aftermath of car accidents and trauma, whiplash also called neck sprain or neck strain, is a soft tissue injury to the neck. Whiplash is characterized by a collection of symptoms that occur following damage to the neck. In whiplash, the intervertebral (located between vertebrae) joints, discs, and ligaments, cervical muscles, and nerve roots might become damaged. It occurs when forceful head movement stresses the muscles, tendons, and ligaments of the neck. It may mimic radiculopathy, with pain traveling down the arm. Pain usually improves rapidly if addressed soon after the trauma, but may become chronic and disabling.
Pinched Nerve
Myofascial pain syndrome refers to a disorder affecting the fascia, or connective tissue that covers a muscle or muscle group. An injured muscle may form a “trigger point,” or a taut fibrous band of collagen. A trigger point is actually a “knot” felt deep in muscle tissue that causes pain in the muscle and in a related limb (so-called referred pain). Many times the muscle becomes spastic and numbness in the arm or hand may occur. Myofascial pain syndrome can be mistaken for a pinched nerve, but the treatment is different. Spasms in the muscles to the side of the neck are strongly related to myofascial pain.

Alleviating pain stemming from inflamed spinal structures in the body is the goal of steroid injection therapy. The injection itself is NOT a cure in the sense that it changes your anatomy by putting discs back into place, decompressing spinal stenosis, or reversing facet joint arthritis. However, it can provide sufficient pain relief, especially during an acute episode of pain, allowing the patient to progress further with physical therapy and rehabilitation programs and use less pain medication.
