My mother was in need of long term care after her second fall at her home. I did not want to place her in a nursing home. She was at Weiss hospital and I was able to get a referral for hospice care. That day a Seasons RN sat with me and my mother was able to be admitted to their program for home services. The nurses and therapists and nursing assistants on the "INDIGO TEAM" were amazing. They gave me support, took care of medication management and my mother was able to pass away in peace and comfort. The team was amazing!
