Female Infertility
Polycystic Ovary Syndrome (PCOS) is a common hormone imbalance in women. Globally 1 in 10 women are affected in some way by PCOS. A third of women wait two or more years before even being diagnosed and 70% of women remain undiagnosed. Delayed diagnosis and treatment are a concern for women in their reproductive years as PCOS is the number one cause of female infertility.

Endometriosis
Endometriosis is a painful, chronic disease that affects 5.5 million women and girls in the USA and Canada, and millions more worldwide. It occurs when tissue like that which lines the uterus (tissue called the endometrium) is found outside the uterus — usually in the abdomen on the ovaries, fallopian tubes, and ligaments that support the uterus; the area between the vagina and rectum; the outer surface of the uterus; and the lining of the pelvic cavity. Other sites for these endometrial growths may include the bladder, bowel, vagina, cervix, vulva, and in abdominal surgical scars. Less commonly they are found in the lung, arm, thigh, and other locations.

A saline infusion sonogram (SIS) is performed to make sure the endometrial cavity (inside of the uterus) is normal. This may be done prior to an IVF cycle or as part of an evaluation for recurrent pregnancy loss or causes of irregular menstrual cycles or chronic pelvic pain.

Women with PCOS may have difficulty managing their weight. Making healthy food choices is an important part in staying within a healthy weight range. A diet low in carbohydrates and higher in lean protein is ideal for both weight loss and low insulin requirement. Choosing foods that have a low glycemic index (GI), are low in saturated fat and high in fiber are preferred. Lowering carbohydrates and limiting processed foods, particularly those with added sugars, can also prove beneficial. A healthy serving of whole grains, fruits and vegetables are key. A weight loss as little as 5% can improve acne, ovulation and fertility, reduce excessive hair growth and improve mental health.

Dr. Sanchez graduated from the University of South Florida in 1991 with a Bachelors of Science in Biology. He subsequently attended the University of Florida College of Medicine, and graduated in 1995 with a Doctorate in Medicine. From 1995 to 1999 Dr. Sanchez performed his Internship and Residency at Bayfront Medical Center in St. Petersburg, Florida, where he received several honors and awards including outstanding laparoscopic surgeon. Following his residency, Dr. Sanchez completed a fellowship in Reproductive Endocrinology and Infertility at the University of South Florida where he presently serves as an Instructor.

Ultrasound
IVF is basically a four-step process. First, you take medications to make multiple follicles begin to develop on the ovaries. This step is referred to as ovarian stimulation, or superovulation. Step two involves monitoring follicular growth by ultrasound, to determine egg growth and uterine lining development. When it is determined that the follicles and the uterine lining are appropriately mature, a trigger shot is then administered. 36 hours after the trigger shot, the third step begins with retrieval of the eggs by ultrasound-guided-needle aspiration, an in-office procedure. A sperm specimen is then washed and prepared for insemination. The washed sperm is then placed in a dish with the eggs, and they are placed in an incubator for 18 hours. After 18 hours, the embryos are observed for normal fertilization. The embryos are then incubated for further development into multi-cell embryos.

Understanding of the risks of permanent amenorrhea in women treated with modern chemotherapy and radiotherapy has changed little over the years. However, there have been outstanding advances in the science of fertility preservation that may affect patient decision-making. The Panel reviewed the new techniques exist to allow embryo and oocyte cryopreservation (with hormonal stimulation), ovarian transposition, surgical options other than radical trachelectomy, ovarian suppression, ovarian tissue cryopreservation and transplantation, and other considerations. Fertility preservation options in females depend on patient age, diagnosis, type of treatment, presence or participation of a male partner and/or patient preferences regarding the use of banked donor sperm, time available, and likelihood that cancer has metastasized to her ovaries. The

Hysterectomy
Conservative surgery seeks to remove or destroy the endometriosis, relieve pain, and often allows pregnancy to occur. Conservative surgery can involve laparoscopy (minor procedure with a tiny abdominal incision) or laparotomy (more extensive procedure, full incision, longer recovery period). Hormonal therapy may be prescribed along with conservative surgery. Extensive surgery, which may be necessary in severe cases, involves hysterectomy, removal of all growths, and removal of the tubes and ovaries.
Laparoscopy
Diagnosis is considered uncertain until proven by laparoscopy, a minor surgical procedure done under anesthesia. A laparoscopy usually shows the location, size, and extent of the growths. This helps the doctor and patient make better treatment choices.

This misplaced tissue develops into growths or lesions, which respond to the menstrual cycle in the same way that the tissue of the uterine lining does: each month the tissue builds up, breaks down, and sheds. Menstrual blood flows from the uterus and out of the body through the vagina, but the blood and tissue shed from endometrial growths has no way of leaving the body. This results in internal bleeding, breakdown of the blood and tissue from the lesions, and inflammation — and can cause pain, infertility, scar tissue formation, adhesions, and bowel problems.
