A hammertoe is mainly diagnosed with a physical exam and a focused history. X-rays can also be used to identify boney prominences that are causing painful corns to form. Blood tests can also be done if there is suspicion of a systemic cause to the deformities, like rheumatoid arthritis.
Plantar Fasciitis
Plantar fasciitis heel pain syndrome can occur via a myriad of causes. Risk factors such as weight gain, foot type (flat feet, high arched feet), a high level of activity, sports, overuse, improper shoegear, improper support of the feet, trauma, tightness of muscles and daily activities can trigger the classic symptoms. Often, there is no single cause, but a culmination of several risk factors. It is imperative to address all of these factors to treat this condition successfully.

After a thorough history and physical we will recommend a variety of conservative treatments for flatfeet. This will include activity modification, weight loss (if needed), orthotic devices, for more severe cases we have ankle foot orthoses, medications, physical therapy and shoe modifications. The mainstays of our conservative approach are functional custom orthotics that we can custom manufacture and prescribe for your specific foot type. They are devices that are placed in your shoes and are not noticeable by others. Orthotics compensate for mechanical faults and allow your feet to function with improved efficiency. Orthotics relieve stress from compromised joints, ligaments, and muscles of the lower extremity.
Pediatric Care
All graduates of a Podiatric college will go on to a two or three year residency program in surgery and medicine. There are many Podiatric physicians that will choose an area of Podiatry to sub-specialize in, including biomechanics of the foot and ankle, Geriatric care, or Pediatric care. Some will continue their education with a fellowship in surgery, sports medicine, or limb salvage. Podiatrists often recognize systemic disease by its manifestation in the foot. Examples of this include diabetes mellitus, rheumatoid arthritis, and vascular disease.

Ultrasound
Diagnostic tests can be performed to confirm the mass is present and to identify the etiology of the mass. An X-ray can show if the long bones in your foot are too close to one another which is causing the irritation and enlargement of the nerve. An MRI or ultrasound of the foot can help identify and locate the mass.
MRI
Plantar fasciitis can be diagnosed by your doctor through a thorough history and physical exam. Ancillary testing, like X-rays, MRI, bone scan, or a CT, can be performed additionally to exclude or include any other boney or soft tissue abnormalities. Ultrasound can be used to confirm the diagnosis. Thickening is noted on the ultrasound due to the inflammation and swelling of the ligament. Additionally, blood tests can be performed to diagnose any arthritic or infectious disorders that can be the cause of the heel pain.

Additionally, he furthered his training in sports medicine, biomechanics, traumatic injuries, and reconstructive foot and ankle surgery at the Carle Foundation Hospital, Carle Foundation Physician Group in Urbana, Illinois. Dr. Gandomani also has experience with treating our veterans while training at the VA Illiana Health Care System in Danville, Illinois.

Corns
Inspect your feet daily. Be sure to check the back of the heel, the sole and in between the toes. Some diabetics find it helpful to use a small mirror to check those hard to see places. Watch for corns, calluses, blisters, cuts, redness–anything out of the ordinary. Always call my office immediately if you discover any problems.
Ulcer
As a diabetic, you have a higher risk of developing foot problems such as foot sores (ulcer) and infections, especially if your blood sugars tend to be above 150. Over time, the elevated blood sugars can lead to diminished circulation, diminished nerve function and can inhibit your body’s ability to fight infection. For example, think about the consequences of stepping on a sewing needle. First of all, if you have a loss of sensation, you probably wont’t feel the needle (so it could remain in your foot for days).
Calluses
A corn is skin that has become hard and thick and can occur on the top, between, or on the tip of the toes. A callus is also skin that has become hard and thick but mainly occurs under the ball of the foot, on the heel, or on the bottom of the great toe. Corns and calluses are often mistaken for skin conditions like warts. In actuality they are a reaction of the skin to increasing pressures which are being placed on those specific sites. These increasing pressures can be due to boney prominences or biomechanical abnormalities when you ambulate. Once these corns or calluses become too thick they place pressure on the underlying structures namely nerves and bursal sacs. This can sensations which will vary from discomfort when walking to extreme pain that will prevent normal walking.
Fungus
Nail fungus, also called onychomycosis, is an infection of the nails which can cause the nails to become yellow, thick, brittle, discolored and sometimes painful. This is a common condition estimated to affect up to 10% of the population.

A bunion is a prominent bump that appears at the side of the big toe joint. This bump represents the movement of a long bone in the foot behind the big toe. As the end of the bone continues to move medially or outward the bump becomes more prominent. As the long bone behind the big toe moves, so does the big toe. It begins to point toward the second toe. Bunions are a progressive disorder and as the big toe joint becomes more characteristic of the letter ‘V’. The apex of the ‘V’ is the bunion prominence and it will point toward the other foot. Over time this can affect the other toes and also create arthritis in the big toe joint. Some never develop symptoms, others have severe pain and can not wear shoes.

Hammertoes
A hammertoe is an abnormal contracture of a joint in your toe due to a partial or complete dislocation of that joint or the joint that connects the toe to the rest of the foot. As the toe continues to contract it begins to rub against your shoe. This can cause your skin to react by thickening to protect the underlying bone, this thick skin is called a corn. But after chronic rubbing this corn can become too thick and also be a source of pain. Chronic rubbing of the contracted joint against the shoe can also cause a bursitis to develop demonstrated in a toe that is red, swollen and painful.
Heel Spurs
Plantar fasciitis, or also know as heel spur syndrome, is an inflammation of the ligament which connects your heel bone to your toes. This ligament is called the plantar fascia. Classic symptoms of plantar fasciitis include pain in the area of the heel near the inside of the foot where the arch of the foot begins. Most individuals feel pain in the morning or after a long period of rest. This is called post-static pain. When you are at rest or not bearing weight on the foot, this time allows the ligament to contract. Therefore when one plants the foot down the ligament will stretch and cause pain. Current thoughts on are that the heel spur that is sometimes associated with plantar fasciitis is not another source of the pain. The spur is simply an extension of bone after the plantar fascia continually stretching and pulling on its insertion on the heel bone.

Dr. Gandomani is a native of Oklahoma and earned his undergraduate degree in Microbiology from the University of Oklahoma. After graduating, Dr. Gandomani completed his medical degree from Western University of Health Sciences in Pomona, California. He then completed his residency in Foot and Ankle Surgery at Jamaica Hospital Medical Center a Level 1 trauma center in Queens, New York. His comprehensive three-year training included complex limb salvage, wound care, foot and ankle trauma, emergency medicine, diabetic foot infections, and inpatient management. Dr. Gandomani’s training also included working with various medical specialties such as vascular surgery, orthopedic surgery, plastic surgery, infectious disease, internal medicine, adult and pediatric emergency medicine departments.

Orthotics are specially designed devices that are worn inside the shoe to control abnormal foot function and/or accommodate painful areas of the foot. Properly designed foot orthotics may compensate for imparited foot function, by controlling abnormal motion across the joints of the foot.
