Plantar fasciitis is typically characterized as a stabbing pain in the bottom of the foot near the heel, but can also be felt along the arch or ball of the foot. The pain is usually the worst in the morning after taking the first few steps of the day. It can also be triggered by long periods of standing or when you get up from sitting.
“But then I had a misstep and a serious fall. As I have Multiple Sclerosis, injuries from falls are much harder to recover from and often leave the patients permanently affected in the ways they can move because of pain and muscle atrophy. Knowing how much I was helped originally I was anxious to return for help and after reluctantly missing one session, I anxiously ‘crawled’ back for more therapy. My therapist listened, understood, and worked out a plan to help me. He patiently explained why the pain was so severe and how I could begin to reduce my pain and improve my whole way of moving.”

MRI
Another one of Mosely’s concepts is called the neurotag. The neurotag refers to how we individually process different stimuli, environments, and situations. It is very important to note that a neurotag is specific only to a particular individual and is very dependent of past experiences. Take for instance someone who has been in a motor vehicle collision. That person very likely has some reservations about driving, especially in particular parts of town or in particular conditions (e.g. snow or rain). When that person is behind the wheel, visible changes can be noticed in how he positions himself and an increase in tonic neuromuscular activity occurs. This increase in tonic activity results in alteration of movement pattern, which can very likely perpetuate their symptoms. Another example of a neurotag involves someone who fell off a ladder at work. If we were able to do a functional MRI of her brain and simply showed her a picture of the ladder in her workplace, it is very likely that the areas of the brain classically associated with pain and fear are activated.
Iontophoresis
Iontophoresis uses either equipment or patch with medicine, lidocaine or dexamethasone, applied to the desired tissues and uses current to administer the medication through the skin
Matthew is originally from Seattle, Washington graduating with a Bachelor of Science degree in Kinesiology from Washington State University. He later went on to receive his Doctorate in Physical Therapy at Midwestern University in Glendale, Arizona. Since beginning his journey into the movement sciences Matthew has gained broad exposure within rehab settings. Working within the exercise science/sports performance laborites at Washington State University, he was able to gain a deeper understanding of athletic performance and injury prevention early on in his education. During his time at Midwestern University, he gained the opportunity to work hands on with a broad range of patients. Excelling in settings from inpatient rehab facilities to outpatient orthopedics and athletic performance centers. As a clinician Matthew applies a whole systems approach, isolating in on not only the root causes, but also contributing factors, in an aim to attain lasting symptom alleviation. Matthew utilizes an approach that includes exercise prescription, biomechanics/neurophysiological education, and manual
Orthopedic Surgery
After orthopedic surgery, PT is needed to rehabilitate the body part affected. Some effects after surgery are stiffness, weakness, decreased ROM, and pain. We at Paradigm progress the patient appropriately to get them back to normal function. It is usually a progression from light exercise for ROM and pain relief to eventual functional exercise for return to full activity. Post operative surgeries that are very common are low back, neck, rotator cuff, ACL repair, and total joint replacements for all body parts. There are also many more not mentioned that we treat.
Knee Replacement
As a physical therapist assistant in my eighth year working in an orthopedic PT setting, I have worked with many postoperative total knee replacement patients. I have always enjoyed working with total knee replacement patients. I have found that most patients have many of the same questions, concerns, and fears once they get to their […]

Manual Therapy
Our treatment approach uses highly specific interventions derived from the evaluative process and based on best evidence of practice. This may include manual therapy, therapeutic exercise, neuromuscular re-education, proprioceptive training, and comprehensive education.
The plantar fascia supports the arch of the foot and absorbs shock when walking. Tension and stress on the fascia can cause small tears resulting in irritation or inflammation from those repeated stresses. Although the cause remains unclear in many cases of plantar fasciitis, ignoring it can result in chronic heel pain that may interfere with your daily activities and altered gait patterns to avoid pain may lead to further foot, knee, hip, or back problems.
