Nutritional Counseling

Comprehensive Examination
There are many heritable diseases in animals. The Canine Eye Registration Foundation (CERF) was established in 1974 to create a database to track heritable eye diseases in purebred dogs. Effective November 2012, the American College of Veterinary Ophthalmologists (ACVO) designated the OFA’s Eye Certification Registry as their endorsed registry (see http://www.offa.org/eye_ecrfaq.html ) A database is maintained through registered purebred dogs examined by board certified veterinary ophthalmologists ( Diplomates of the ACVO — American College of Veterinary Ophthalmologists ). This database helps breeders and ophthalmologists monitor eye diseases. A dog that is registered by OFA ECR has been examined by an ACVO diplomate and found to be unaffected by a major heritable eye disease. OFA ECR examinations are performed annually. The ophthalmologist fills out the OFA ECR form and gives a copy to the owner. This copy can be sent to OFA by the owner (if the dog has been found to be unaffected, as previously described) along with the registration fee to receive an OFA ECR registration number, which can be used by the owner for show purposes, breeding, and AKC pedigrees. Registration is good for one year and it must be renewed annually by examination. Many ocular diseases do not appear until later in a dog’s life, such as PRA and some forms of cataract, so annual examinations are necessary to help rule out heritable diseases. The earlier that these diseases are diagnosed, the better. Some inherited diseases, such as PRA, can be identified via DNA testing. Please visit http://optigen.com/ for more information. Illness Treatment

Pain Management
Proptosis is another frequent eye injury in dogs, particularly flat-faced dogs. Pugs, Shih Tzus, and Pekingese all have flat faces and shallow eye sockets. The shallow sockets and the large eyelid openings make them high risk for proptosis occurring (see Macropalpebral Fissure Syndrome ). An eye forced from the eye socket is an emergency, and must be replaced in the eye socket by a veterinarian as soon as possible. Until you can reach a veterinarian, keep the eye moist with sterile saline solution (NOT cleaning solution), eye irrigating solution, or (as a last resort) water. If you have any artificial tears, this also can be helpful, but it is important to keep the tissues moist and the patient quiet. Do not give aspirin or any other pain medication — let the doctor decide what type of analgesic to give. Do not let the dog rub at its eye, and take away its food and water, as the dog will likely be anesthetized in order to have the eye replaced in the eye socket. Even after the eye is replaced, there is a significant risk that the eye will not recover vision, and might eventually require surgical removal (enucleation). Sometimes the proptosed eye can be saved, but needs plastic surgery to protect it (medial canthoplasty).

Infection Treatment

Feline Retro Virus Treatment
In approximately 6 out of every 10 cats with uveitis, the cause cannot be identified, even with extensive diagnostic tests. However, when the cause can be identified, it is usually trauma, an infectious disease, or cancer. The most common infectious diseases that can cause uveitis in cats are: Bartonella henselae, feline infectious peritonitis (FIP), Toxoplasma gondii, feline immunodeficiency virus (FIV), feline herpesvirus-1 (FHV-1) and feline leukemia virus (FeLV). The two most common types of cancer that can cause uveitis in cats are lymphoma and also diffuse iris melanoma (DIM). Discussing these various diseases is beyond the scope of this handout. It is important to know that all of these diseases can be life-threatening, with perhaps the exception of feline herpes virus.
Bacterial and Viral Infection Treatment
Cats may benefit from special oral nutritional antioxidant supplementation to strengthen immune function. Supplements that modulate the immune system ( Imuno-2865 ) and that help alleviate epithelial damage from viral infection ( SHaNa-Vet ) may help in supporting cats with FHV-1 infection, as long as administration is not stressful (such as mixing the contents of the capsules in food rather than pilling your cat). Imuno-2865 contains natural beta glucans derived from mushrooms and grasses, that support immune function. SHaNa-Vet contains the natural plant extracts docosanol and triacontanol, which belong to the family known as policosanols, and are predominantly found in naturally occurring phytowaxes such as rice bran and sugar cane waxes, as well as beeswax.

Diabetes Treatment
The second-most common cause of cataracts in dogs is diabetes (diabetes mellitus). 75% of diabetic dogs will develop blinding cataracts within the first nine months of being diabetic. Diabetic cataracts develop VERY fast—often overnight—in dogs, and they are a medical and surgical emergency. Laser Surgery
After successful cataract surgery dogs see close to normal. However, we cannot give dogs perfect vision. This is because an exact replacement of the original living lens is not possible. Furthermore, dogs have more inflammation in their eyes after surgery than humans and therefore have more scarring. This scarring does slightly decrease vision. The post-operative inflammation and scarring (lens capsular opacification) may be lessened by both anti-inflammatory drugs and oral antioxidant supplementation ( Ocu-GLO™ ). Capsular opacification is especially a concern in puppies and young adult dogs, and the best way to help reduce the risk of opacification is to implant an IOL, control uveitis, and provide daily supplementation with Ocu-GLO™. Some of the ingredients in Ocu-GLO™ work synergistically to help reduce the risk of opacification. Once the capsule develops opacities, they cannot be medically reversed. In people, capsular opacities that develop following cataract surgery can be lessened with laser surgery, but this does not work in dogs. Most owners notice a tremendous increase in their pet’s vision after cataract surgery, but they can still detect certain visual difficulties.

Laboratory Services
There are DNA blood tests available, to determine if dogs are likely affected with PRA, are likely carriers for PRA, or are not likely carrying the PRA gene.
