        avoid this place at all time. They do not ask for your insurance card then they will try to ask collection agency to come after you for $25 . even you provide them with right insurance they will ask you to pay them directly and then submit the claim directly to insurance.I spent over $1000 in this place and they went after $25 which was their fault
