Although thyroid disease is less common in children compared to adults, occasionally surgical issues arise. Most commonly this is for an overactive thyroid gland (hyperthyroidism) or a concerning thyroid nodule. Treating pediatric thyroid disease requires a multidisciplinary approach. As the lead surgeon of our Thyroid Surgery Program, Dr. Horwitz works closely with our partners in pediatric endocrinology, pediatric radiology and pediatric pathology. In addition, because recent studies have suggested that high volume thyroid surgeons (performing > 30 cases/year) have improved outcomes, Dr. Horwitz has partnered with Dr. Teresa Kroeker, one of the regionâ€™s leading adult thyroid surgeons. This collaboration between pediatric surgeons and a high-volume adult thyroid surgeon optimizes the care of children with surgical thyroid disease conferring the benefits of both a high volume thyroid surgeon and high volume pediatric surgical care.
Hypothyroidism
The thyroid gland is a butterfly shaped organ found in the neck. The thyroid gland produces thyroid hormone which regulates the bodyâ€™s metabolic rate as well as heart and digestive function, muscle control, brain development mood and bone maintenance. Problems with the thyroid gland include thyroid nodules (abnormal growths), hyperthyroidism (overactive gland), and hypothyroidism (underactive gland).

Thyroid nodules are commonly diagnosed with a physical examination and an ultrasound. The concern with thyroid nodules in children are that they may harbor a thyroid cancer. A needle biopsy may be recommended. Hyperthyroidism and hypothyroidism are usually diagnosed by blood tests. Most children with thyroid problems are first referred to a pediatric endocrinologist.

Ultrasound
The diagnosis of an inguinal hernia is usually suggested by the history of a groin bulge that comes and goes.Â The hernia can be confirmed by a physical examination, and our surgeons rarely require additional testing. If any question exists about the presence of an inguinal hernia, an ultrasound can be performed to assist with the diagnosis.
MRI
Blood tests specific for IBD have been devised and are often utilized but their accuracy in confirming the diagnosis is mediocre. X-rays, CAT scans, and MRI scans may be helpful to determine the extent of the intestinal involvement and whether there has been narrowing or blockage from Crohnâ€™s.Â

There will be regular visits with the oncologist for tests (blood work, ultrasound, or CT scan) to make sure your child remains healthy and the tumor is not returning.
Chemotherapy
Based on the results, the multi-disciplinary team will recommend the next step. The first step in treatment is usually surgery to remove all the involved tumor. This is called a radical nephrectomy. This involves removing the kidney with the tumor, the ureter (tube connecting to the bladder) and tissue surrounding the kidney with lymph nodes. Rarely, the tumor is so large or involving both kidneys and chemotherapy will be given before surgery.
Radiation Therapy
The definitive treatment of the tumor will be dependent on the specific diagnosis. It may involve observation, surgical excision, radiation therapy or chemotherapy. If surgical intervention is warranted, we will use minimally invasive techniques when appropriate and feasible.

General Surgery
Pediatric surgeons specialize in the care of infants, children, and adolescents. Following graduation from medical school, a surgeon must complete at least five years of graduate surgical education in a general surgery residency program. Two additional years in a pediatric surgery fellowship complete the required training for a pediatric surgeon.
Wound Care
Instructions for wound care and postoperative activity will be given to you by your surgeon. Postoperative pain can normally be managed with acetaminophen (Tylenol) and/or ibuprofen (Motrin). Your child will be scheduled for a follow up appointment 2-4 weeks after the procedure.
Minimally Invasive Surgery
Dr. Rana is a clinical pediatric surgeon with experience in all aspects of neonatal and pediatric surgery, with special clinical interest in pediatric oncology, thoracic surgery and complex gastrointestinal surgery including minimally invasive surgery. He is also the Educational Experience director.
Laparoscopy
Austin Pediatric Surgery has specialized knowledge and training in the surgical repair of birth defects, some of which are potentially life-threatening without intervention. We provide surgical services for the neonatal intensive care units throughout Austin. In many cases, even in these newborn babies, laparoscopy and thoracoscopy can be utilized to correct their conditions and hasten their recovery.

All families will start with a consultation with a pediatric surgeon. If the decision for surgery is made, we employ a joint evaluation with pain control specialists and physical therapists around the time of the operation.Â We have a directed physical therapy program that begins before the procedure and continues after surgery.Â Patients are generally out of bed the day of surgery and most can be comfortably discharged in the first few days after the operation.
