The most common cause of irregular or heavy bleeding is irregular ovulation. If there is no ovulation your period may be late and when it comes it may last a long time or be unusually heavy. Irregular ovulation is more common if you are underweight or very overweight. If you are overweight, this increases your risk of heavy and irregular periods. If this goes on for many years it may increase your risk of uterine cancer later in life. If you are overweight you may benefit from a low-sugar, low-calorie diet, weight loss and exercise for at least 30 minutes per day.

Heavy and/or irregular bleeding in a young woman requires a complete medical and menstrual history as well as a complete physical exam. A complete list of medications is important. Blood tests should be performed for anemia, thyroid disease, pregnancy and prolactin level. Sometimes more hormonal tests are necessary to rule out other rare causes of absent or erratic periods. A cervical culture for infections like gonorrhea or chlamydia and a Pap smear may need to be done. An endometrial biopsy or a D&C diagnose pre-cancerous and cancerous lesions of the uterus. Ultrasound may be needed to look for uterine or ovarian tumors that are too small to be felt on an examination. If periods and/or intercourse are painful, a laparoscopy may be needed to diagnose endometriosis.

Ultrasound
Accurate dating of the pregnancy is very important. We advise an ultrasound during the first trimester. This will be repeated at twenty weeks or so to check for normal fetal growth.

Robotic Surgery
There are the usual risks for surgery and anesthesia including infection, injury to internal organs and bleeding. The robotic approach likely has less risk than traditional hysterectomy because of the minimally invasive nature of the surgery. The robotic way may take longer to perform and requires a specially trained surgeon. All of our physicians are certified in robotic surgery.
Hysterectomy
Robotic assisted laparoscopic hysterectomy is a minimally invasive way to remove your uterus and possibly your ovaries. There are 4 small (approx 1 cm) incisions made on your abdomen. The robotic arms are attached through ports in those incisions. Then the surgeon controls the robot to do complex surgery in a minimally invasive fashion.
Endometrial Ablation
Treatment depends on what cause for the irregular or heavy bleeding was found. It also matters what your plans for a future family are. If there is no obvious cause, there are many options including birth control pills, injectable hormones, the Mirena® IUD, endometrial ablation, and uterine artery embolization.
Laparoscopy
Dr. Lorraine Novas is a graduate of Northwestern University and the University of Illinois Medical School. She completed her residency at Michael Reese Hospital in Chicago. She has been practicing since 1991 and is a Board Certified Physician. Her office is centrally located in the northwest suburb of Arlington Heights. Her areas of special interest include laparoscopy, vaginal hysterectomy, preconceptual counseling and ultrasound technology.

Laparoscopic Surgery
All of our doctors are proficient at Robotic Surgery. Northwest Community Hospital is an epicenter for Gynecological Robotic Surgery. This exciting technology allows us to perform surgery that previously required large incisions with very small incisions and less blood loss. Patients often go home the same day they have their hysterectomy and with less pain. We also perform laparoscopic surgery and hysterectomies through a vaginal incision. These techniques similarly offer rapid recovery with less pain and bleeding than traditional open surgeries.

Your first visit includes a complete physical exam. If you have not had a recent pap test, which is a screen for precancerous lesions of the cervix, it would be done at this time. A cervical swab to test for gonorrhea and chlamydia is also standard at this exam if it has not been recently done.
