Northgate Health and Rehabilitation Center
My Mother was placed her by the Case Manager at the hospital while I was out of town and I was really scared because I was able to screen the facility first. She was there 3 days when I was finally able to go see her. I was surprised to see her in the rehab gym working with the therapist. When she was finished we were asked to attend a care conference. We were asked about what she likes to eat, how she like to spend her time, down to what time she likes to take her bath. These people are fantastic, they really care about her happiness. I couldn't have chosen a better place for my Mom. Monica the RN stays on top of everything.

Whoo-hoo for Northgate! A family member reported to ADON, see comes to see her father every day in the evening. She stated, "I am well pleased with the care and the staff on 400 hall. Brandon is a great nurse, Dymon and Sheena are some of the compassionate CNA"s I know!'

Mesa Vista Inn Health Center
I use to work for Mesa Vista, but I had to quit because they ALWAYS had us working short staffed in the C.N.A. department. When I say short I mean 2 C.N.A's for 45 to 50 people, and the Director of Nurse and the Administrator would know all about it. After I resigned from Mesa Vista they started accusing me of stealing medical supplies, and on top of that I called the Regional manager for Mesa Vista to let her know how the place is being ran and she went of the defense as if I was talking about her personally( Ms. Karen Mc Quay). She wasn't interested at all in how the patient's are being cared for. They need a new D.O.N, Administrator, and Regional manager cause they don't Care about the patient's at all. They Know I'm Right To. Yours Truly. RUBEN. I would give them 0 star rating if it was possible.

My mom lived in the memory care section for almost 10 months. The first couple of months went well, but things went down hill quickly. They stopped bathing her, brushing her teeth and her room began to smell like urine. I asked many times for someone to take care of these basic needs, but nothing changed. The staff left my mom in a wooden chair in the living room all day, every day. This wore a bald spot on her head and left an indentation as well. During the last 7 months she fell 8 times. Nothing was done after any fall to prevent another one. The last fall left the entire side of her faced bruised. Ken, who is suppose to be In charge of care plans never made an effort to implement any changes to prevent her falls. His attitude was unprofessional, offensive and aggressive. I told Debbie, the executive director that I didn't feel safe leaving my mom there any longer, but she still wouldn't waive the 30 day move out notice. Debbie and Ken showed no empathy for my mom. I would not recommend this location to anyone. I ended up finding a much higher quality home that ended up costing over $1,000 less per month.

Westover Hills Rehabilitation and Health Care Center
The staff is very slow to aid the residents. One time I went to see my mother and she smelled so badly because her pants were soiled and the bathroom walls and toilet were covered in feces. You could smell it coming down the hall so I know the aides could too. The floors were always dirty. Within a two year span I watched it go downhill very quickly.

Stone Oak Care Center
The bottom line is making a profit and not caring for the residents there, the state is always there and yet nothing is found! Bed sores, baths once a week instead 3 a week. Activities are for the low function residents. What about the one who still have their minds. The activities director is always in her office and never around. So they leave the volunteers out there and they don't know what to do. I wanted to take my mother there but I was told don't, we are going elsewhere!

This Facility is a Hell.. Need i say more!! They do not supervise there patients/ residence. And staff or Nurses haveno clue how treat them. Or give them adaquit care what so ever. Or HYGIENE!! Family member fell out of his bed and was left for hours!! Facility is trying to cover it up. This Facility should be investigated toi full extent with so many horrible reviews.. I give no stars i give nothing to this establishment...but a death wish for your loved one!! The 1 star on professionalism goes to the therapy gentleman who gave my loved one a brief session of physical therapy and wished him well as the facility booted him after the fall..They did not want to care for my family member and told me so. Again do not go hear. It was close by... But REGRET IT terribly!!!!!!

Mesa Vista Inn Health Center
I placed my mom at that nursing home and I fell in love with the staff they were all great to my mom they went above and beyond to take care of her. She is on hospice and was there for respite care which means she was only there for five days. But I recommend a place to everyone that I know that is on hospice. I have placed my mother in different locations but she has never had the best care other than Mesa Vista. So thank you Mesa Vista staff for taking good care of my mom and having her participate in a lot of activities during her five days stay. Patricia G
