Medical Weight Loss
The shape of the breast is determined by the consistency of the breast tissue and the skin tension. This can change with time, breastfeeding or weight loss. In some people, the breast tends to sag without any reason. Many times after pregnancy, the breast loses volume and becomes hollow on the upper pole, however, the nipple is still above the infra-mammary fold. This condition is called pseudo ptosis and can be corrected with a breast implant, which reestablishes the volume.

Endoscopy
The Brow Lift is necessary when the eyebrows are too low, giving the patient a tired, unhappy, or angry look. It was traditionally done through a coronal approach, which necessitated an incision a few inches behind the hairline, or if the hairline was too high, an incision in front of the hairline. The scalp behind the incision remained numb. In the early 1990s, we adopted the endoscopy in plastic surgery. I have been doing endoscopic brow lifts since 1993.In the beginning, we freed the soft tissues of the forehead from the underlying bone, lifted the brow up and maintained it by screws, and later by absorbable hooks (Endotine). This was a lot less invasive and preserved the nerves to the scalp, but the longevity was not as good as the standard coronal lift.

Ultrasound
These techniques allowed the operation to be done with very little blood loss and no need for transfusion which was common with the earlier techniques. This was a major advancement, but like the liposuction itself, was very low tech and ingenious. The other advances, like the addition of ultrasound or laser, are not so important and have few applications. There is an enormous push from the industry to make the laser liposuction appear as if it were a state-of-the-art procedure. These manufacturers are conducting a huge marketing campaign to convince every plastic surgeon to buy their machine. At this time, there is no proven benefit for the laser or even the ultrasound liposuction and I believe they are not worth the additional cost, danger and complication rate.
Chemotherapy
The patient is seen before the operation and marked for the pattern of the mastectomy. After the mastectomy, the pectoralis muscle insertion on the chest wall is divided and a space is created below the muscle. A piece of Acellular Dermal Matrix (ADM) is sewn to the chest wall in the area of the infra-mammary fold and the lateral breast fold. The expander, or in some cases the implant, is placed under the muscle, supported by the ADM from below. The ADM is then wrapped over the implant and sewn to the free edge of the muscle. The expander or the implant can be filled almost to the maximum volume. By saving most of the breast skin during the mastectomy, one can achieve a natural look to the breast immediately. The patient usually stays a night in the hospital. Drains are removed in about five days. Several months later, when the chemotherapy or the radiation is completed, the final stage is performed. This operation consists of symmetrization and the nipple reconstruction. The availability of ADMs and the very appropriate anatomic shaped, form stable implants available today have made the prosthetic breast reconstruction so successful that autologus reconstruction is rarely needed and is reserved for complicated post radiation cases.

General Surgery
Dr. Rafizadeh completed a pre-medical degree at Neuchatel University in Switzerland before graduating with his medical degree from Lausanne School of Medicine in Switzerland. He then came to the US to complete his postgraduate training, including a General Surgery residency at Saint Barnabas Medical Center and Chief Residency at Morristown Memorial Hospital. To further advance his training, Dr. Rafizadeh completed a fellowship in Plastic Surgery at New York Hospital, Cornell Medical Center (New York Presbyterian) and Memorial Sloan Kettering Cancer Center.
Ear Surgery
Otoplasty or ear surgery is a general term that can include and ear lobe repair or ear pinning or even a complicated reconstruction of a congenital ear deformity.

Plastic Surgeons

Botox
The course of the facial nerve branch to a particular muscle is traced using an accurate nerve stimulator. Then the probe is placed on top of the nerve. The area is anesthetized with a local anesthetic and then heat is applied to affect the nerve and prevent conduction. The result is very much like Botox, but it lasts longer, 6 months to 2 years depending on the number of areas where the nerve is treated.
Brow Lift
I operate at Morristown Memorial Hospital, the Morristown Surgery Center next door to our office, or at the Florham Park Surgery Center. I feel that it is important to perform these procedures in a state-of-the-art facility with good support even if the surgery is done under local anesthesia. I hope this brow lift information has shed new light on treatment and procedures.
Eyelid Surgery
I have developed a method of administering local anesthesia, which makes it possible to use mild sedation to relax the patient and have their cooperation during the surgery, especially during eyelid surgery.
Liposuction
During the arm lift procedure, an incision is made along the inside of the upper arm, often spanning from the underarm to the elbow. Your surgeon will then remove excess fat using liposuction, before trimming and tightening the excess skin and suturing it into place. For patients with only a small amount of excess skin, a minimal incision arm lift procedure may be performed, in which a few small incisions are made near the underarm area for a less invasive fat and tissue removal. Dr Rafizadeh will determine which procedure is best for you after a physical exam and a discussion of your individual goals.
Breast Augmentation
Dr. Rafizadeh strives to achieve a balance between what the patient wants and what is a safe, aesthetic augmentation. He bases the decision and the discussion on the patient’s dimensional requirements. Dr. Rafizadeh carefully analyzes patients during their first consultation and makes recommendations about the size and shape of the breasts. He takes time answering all questions regarding breast augmentation, and the choice of breast implants. In addition, Dr. Rafizadeh provides his patients with real breast implant samples so that the patient can see how they will look before undergoing breast augmentation.
Breast Reduction
Breast reduction also called reduction mammoplasty is a variation of mastopexy, when breast tissue is removed. The same patterns can be used as in mastopexy. Most often, we use the vertical or the inverted T pattern or anchor pattern. In large breasted individuals, several pounds could be removed from each breast. The nipple areola is raised to a higher position carried by a pedicle of tissue giving it the blood and the nerve supply. The scars would vary in length depending on how large the breasts are. The larger and more saggy the breasts are the longer the scars need to be to achieve optimal shape.
Breast Lift
Breast lift (ptosis correction or mastopexy) is done by reducing the excess skin of the breast. In mild cases, this can be done by removing skin around the areola. This is called the periareolar or doughnut mastopexy. This was first described by a French plastic surgeon by the name of Benelli, therefore it is also referred to as the Benelli lift. I like this operation because it produces the least amount of scars, but it’s only OK for mild cases of ptosis; otherwise the results are not stable over time and stretching of the areola can be a problem. This operation, like other mastopexy procedures, can be combined with an augmentation.
Rhinoplasty
Dr. Rafizadeh conceptually divides the nose into an upper part (which is bony) and a lower part (which is made of cartilage). The bone forms an arch that makes the nasal bridge. Below this area, the upper lateral cartilages form a cartilaginous arch that is a continuation of the bony arch. The shape of the tip is related to the lower lateral cartilages that are a pair (shaped like an inverted U). Dr. Rafizadeh’s experience and specialization in Rhinoplasty help him achieve outstanding results.
Breast Reconstruction
The other aspect of breast reconstruction is the symmetrization of the opposite breast. In many cases, the opposite breast is too small, too big or too saggy. In these cases, something is done to the other breast to improve the symmetry. This is done as a second stage procedure, during which the nipple and areola are reconstructed.
Plastic Surgery
Here is a great example. Do you remember this beautiful french icon, BeBe or Brigitte Bardot? She had the most beautiful face. I don’t think that Brigitte Bardot has had much plastic surgery in her life, but one thing is sure, she has lived and we can see what time can do to us by looking at her picture. The effect of aging on the upper lip is striking. She had the sexiest short and upturned upper lip, showing her incisor teeth when she was young and now they are covered by an elongated upper lip. When I see a patient for facial rejuvenation, I try to mention this and we have done many upper lip shortenings and every time the patients are ecstatic with the results. Now the procedure has become more popular and we have more demands for it. Today patients travel from as far as California and Canada to have the procedure performed here in Morristown NJ.
Thigh Lift
Light activity is normally possible after less than two weeks, while most physical activity is permissible at four to six weeks. All symptoms should be gone and scars maximally faded within six months. After recovery, patients will be able to show off their slimmer, more defined legs and eliminate any previous embarrassment. The results of a thigh lift can be permanent, as long as patients maintain an active and healthy life and do not gain significant amounts of weight. Additional surgery may be performed if this occurs.
Face Lift

Neck Lift
This is an alternative to a face and neck lift and is good for a patient who does not want the extra recovery time and the extra cost of a facelift. I would recommend this procedure for a patient with early signs of skin laxity who is not yet ready for a facelift. The arm lift is different because to accomplish these results surgically, there would be a long scar on the inside of the arm that would be visible. In this case this procedure offers a definite advantage over the surgery.
Blepharoplasty
Candidates for this procedure can be as young as 30 years but typically they are in their 50s and more in age and have excess upper lid skin and baggy appearing lower lids. They often appear tired and request the procedure for the improved appearance. There are, also patients who have such excess in the upper eyelid skin that hangs beyond the lid margin and causes some difficulty in their visual field, especially in the upper outer gaze area. Those patients will request the blepharoplasty for medical reasons because they have trouble seeing their sides. The cosmetic improvement that this procedure will give them is a secondary benefit. In these cases when an ophthalmologist measures the visual fields and proves the problem, the insurance might pay for the procedure.
Abdominoplasty
What is a mini abdominoplasty or the mini tummy tuck? To understand the mini tummy tuck one needs to know what a standard tummy tuck is all about. The standard tummy tuck involves an incision above the pubis, which typically goes from hipbone to hipbone on the other side. The skin of the abdomen is lifted over the muscle layer and the muscle separation that usually happens with pregnancy is repaired over the midline. The excess skin below the belly button is removed and the skin is closed above the pubis. The belly button is then brought out through a hole that is made in front of it and is repaired to the abdominal skin. Two drains are placed to allow the skin to heal with the deep muscle layer. With this operation, all of the excess skin both from above and below the belly button is eliminated.
Body Lift
This procedure may be performed alone, or in combination with other cosmetic procedures, such as part of a body lift or with a buttock lift. When combined with other procedures, patients can achieve maximum results and enjoy a younger and more refreshed appearance overall. Breast Surgery
The scars are the main side effect of any breast reduction procedure. Dr. Rafizadeh has the biggest experience in breast surgery and knows to to keep the scars the shortest possible and still achieve the best breast shape. There could also be some reduction in nipple sensation especially if the nipples have to be moved a long distance from the original position.
Otoplasty
Lip Augmentation
In general, the longevity is dependent on the product, but is also dependent on where it is used. The less mobile an area is, the longer the product lasts. For example, Restylane could last for two years or more in the tear trough, but only six months in the lips, where there is a lot of motion. It also depends on the patient. In some patients, I have used Restylane for lip augmentation and I didn’t have to redo it for more than two years. In others, it lasts less than six months. Patients break down these products at different rates, and the only way to find out is to try.
Body Contouring
Today, liposuction is the most common plastic surgery procedure. For me, the liposuction cannula is a tool that I use for many procedures, from reconstructive to cosmetic, from facial surgery to the surgery of the breasts and body contouring. Liposuction is a great procedure that removes the fat and preserves the nerves and vessels. The fat can be used for grafting to other areas. It has been shown that the fat removed by liposuction has stem cells that can help reverse the bad effects of radiation.
Gynecomastia
Male Breast Reduction or Gynecomastia refers to excessive breast tissue in a male patient. This can be unilateral or bilateral affecting both breasts or just one side. This is perceived as a serious deformity by young men who come to the plastic surgeon to seek treatment. Some degree of excessive breast development, associated with some tenderness of the nipples happens at puberty in most boys, but this resolves spontaneously over time. Presently, Dr. Rafizadeh ’s practice is focused on cosmetic surgery of the face, breasts, liposuction and reconstructive breast surgery after mastectomy. He has served as Chairman of Plastic Surgery at Morristown Memorial Hospital for many years, and has witnessed the transformation of Morristown Memorial into one of the most important tertiary care centers in Northern New Jersey. Facial Implants
To turn the clock back and establish an appearance of youth, we ideally need to reverse all these changes. Face lifting alone will counter laxity, which was caused by the loss of elasticity. Therefore, we tend to reverse laxity, knowing that we can’t reverse the loss of elasticity. We try to reposition the soft tissues to their original position. We can counter the effect of fat loss and bone loss (loss of volume) by fat grafting, facial implants or simply injectable fillers. We can also restore the skin surface by resurfacing and skin care. True rejuvenation requires attention to all of these issues.

Skin Care
Injections are a routine office procedure, best done by a plastic surgeon who is familiar with the anatomy of the face. While many doctors offer these injections, only a select few have the qualifications and training required to perform them effectively. Dr. Rafizadeh not only has the experience and training necessary, but also carefully informs patients about the difference between various neuromodulators and fillers. Neuromodulators such as Botox and Dysport eliminate muscle function in a specific area to remove wrinkles. Fillers on the other hand, add volume to the face, fill depressions or wrinkles, or create and enhance facial features like the cheekbones and chin.
